Use this form to provide donations/sponsorships to the KPA if you need to use a credit card. This would be used for things we do not have a specific form for (ex: donations/sponsorships for the 5K, one off general use donations, etc).  There is not an option to leave a note on this form, PLEASE send the KPA an email with what these funds are for.
